Custom Materials Request

This page is used as a resource for orders that require material requirements outside of normal specifications and should only be used when required by Lewis Cabinets. Simply fill out each required field with the material information (Manufacturer, color, finish, core, etc), if you have multiple material schedules on one order, you can add a new group by clicking the “plus” button in the lower right-hand corner. Click submit once finished, and a representative will send a confirmation.
